At MountainView Pediatrics we want your visit experience to be a positive one from start to finish. As busy parents, we understand that your time is valuable. We have listed our office policies for your review in order to help make your child's visit as efficient as possible.
It is helpful if you can arrive 15 minutes early to fill out necessary paperwork. Or, you are welcome to download the registration papers here and bring these completed to your appointment. Paperwork must be completed by a parent or legal guardian. We require a copy of your insurance card and drivers license. We will ask that a parent or legal guardian sign a "Release of Records" to obtain any records from previous physicians. Please bring the name and address or phone number of previous physicians if at all possible.
Co-pays are required to be paid at each visit regardless of who is bringing the child in to the office. While we understand that many of our patients live in blended families we cannot bill the parent or guardian who is responsible for medical expenses. It will be your responsibility to collect from any 3rd party.
